• I/we confirm that all treatments and procedures are undertaken in designated treatment areas.
• I/we confirm that all disposable sharp instruments are disposed of in a clinical sharp instrument container.
• I/we obtain a consent form from all clients which contains the date of birth, full name of the client, date of the procedure, and the details of the therapist carrying out the treatment.
• I/we follow infection control, sanitation, and sterilisation protocols.
• I/we perform patch testing and pre-procedure evaluations on every client.
• I/we do not perform any manual activities away from the premises to which this application applies.
• At my/our premises, I/we only perform activities associated with the normal running of a beauty, aesthetics, and cosmetics practitioner.
• I/we do not sell or supply any product which is not (a) manufactured in the United Kingdom, European Union, or United States of America; or (b) printed material.
• I/we do not perform any work using heat or naked flames (other than as required in the normal provision of beauty, aesthetic, and cosmetic treatments).
• Where a procedure requires aftercare, I/we provide the client with aftercare advice in writing.

• I/we undertake a patch test prior to the use of any laser (as per laser condition).
• I/we record the results of the patch test on the client’s record.
• I/we have appointed a Certificated Laser Protection Adviser.
• I/we confirm that the Certificated Laser Protection Adviser’s RPA2000 qualification is current and I/we have retained a copy for our records.
• I/we have implemented Local Rules and confirm that these are reviewed every 12 months as a minimum.
• I/we confirm that all laser equipment is used, inspected, maintained, and (re)calibrated in accordance with the manufacturer’s guidelines.
• I/we hold a Special Treatments Licence where such licence is required.

Certificated Laser Protection Adviser means a person who holds a Laser Protection Adviser Certificate issued by RPA2000 or the Association of Laser Safety Professionals, appointed by you or on your behalf, who has day-to-day responsibility for maintaining safe laser use.

Local Rules means written safety information and procedures which set out the safe working practices for the use of lasers, IPL systems, and LEDs, set by the Certificated Laser Protection Adviser.

• I/we have never been convicted, cautioned, or charged with any offences (other than motoring convictions and spent convictions under the Rehabilitation of Offenders Act 1974).
• I/we have never been declared bankrupt, been a director of a company that has gone into liquidation within the last 10 years, or had any personal or business-related CCJs.
• I/we have never had any insurance policy declined, cancelled, refused, or had any special terms imposed.
• I/we are domiciled within the UK and only carry out work within the UK.
• I/we are not aware, after reasonable enquiry, of any circumstance or shortcoming in my/our business activities that is likely to give rise to a claim against me/us, including any adverse reaction, pain or scarring, verbal or written complaint, or refusal to pay for a treatment or procedure.
